Boyle to conduct two special forums at KleinLife to assist residents with state forms
PHILADELPHIA, Jan. 19 – State Rep. Kevin Boyle, D-Phila./Montgomery, will host two special forums to help area residents with filling out forms and services offered by the Commonwealth of Pennsylvania.
The forums will be held on Thursday, March 3 and Wednesday, March 9 from 9:30 a.m. to 11:30 a.m. at KleinLife, 10100 Jamison Avenue.
Boyle’s office will assist residents with forms and application such as: the 2015 property tax rental rebate, LlHEAP (Low Income Heating Assistance Program), PACE applications, medical assistance applications, SNAP applications, reduced senior vehicle registration, handicapped placards, PENNDOT issues, birth and death certificates, and unclaimed property applications.
In addition, they can provide information on living wills, how to obtain a free credit report, senior directories, and 2015 PA tax booklets and tax forms.
The public is asked to make an appointment in advance by calling his district office at 215-331-2600 in order to attend. Those responding prior to February 24th will be invited to join Klein center members for lunch.
